We understand that by participating in the SHARE Benefit Golf Tournament (tournament) to be held on September 26 & 27, 2026, we do so at our own risk. This includes, without limitation, our use of the facilities and grounds of the Lions Club Municipal Golf Course and Mystic Creek Golf Course, equipment, parking areas, sidewalks, as well as our participation in any activity, class, program, or instruction associated with the tournament (tournament facilities). We agree that we are voluntarily participating in these activities and using the tournament facilities and assume all risk of personal injury, illness, including death, and any damage or loss to us or our property that might result therefrom, including without limitation, any loss or theft of our property. For and in consideration of the use by us of the tournament facilities, we agree on behalf of ourselves and our personal representatives, heirs, executors, administrators, agents, and assigns to forever release, discharge, indemnify and hold harmless the Tournament Committee, SHARE Foundation, First Financial Bank, other Tournament Sponsors, the Lions Club Municipal Golf Course and Mystic Creek Golf Course, their directors, officers, employees, agents, representatives, successors, and assigns from any and all losses, claims, demands, damages, or causes of action, known and unknown, we may now have or may ever hereafter have, resulting from or arising out of our use of the tournament facilities and participation in the tournament. This Waiver of Liability and Indemnity includes without limitation injuries or damages which may occur as a result of our use of any tournament facilities, or our transportation to or from the tournament facilities.

RELEASE TO USE PHOTOGRAPHS: We grant to the tournament and its representatives the right to take photographs of us and our property in connection with the tournament. We authorize the tournament, its assigns and transferees to copyright, use and publish the same in print and/or electronically. We agree that the tournament may use such photographs of us with or without our names and for any lawful purpose, including for example such purposes as publicity, advertising, and web content.

INCLEMENT WEATHER POLICY: Once play has begun, no refunds will be made due to inclement weather.
